NEW MEMBERS JOINING AFTER NOVEMBER 30, 2011will be charged Guest admission fee to the December Membership Appreciation Dance

COMPLETED MEMBERSHIP/LIABILITY WAIVER FORMS ARE REQUIRED, ANNUALLY
New membership cards will not be mailed, unless specifically requested
Membership cards DO NOT EXPIRE - PLEASE DO NOT TOSS YOUR CARD
THERE IS A $3.00 ADMINISTRATIVE FEE FOR A DUPLICATE
Sticker for current year's membership will be attached to the back of current card
Card (for new members) or renewal sticker (for prior members) will be available at the next dance attended
Confirmation of receipt of dues will be provided by email, if requested
All memberships are for the calendar year and expire December 31st
